Introduction


The Vessel Documentation Transmittal Letter is designed as a cover sheet when mailing, faxing, or emailing applications and other items to the National Vessel Documentation Center. This is important so they will know your intention and how to make contact in case of any deficiencies. The actual Certificate of Documentation will be mailed to the address shown on an application for initial or re-documentation regardless of that shown on the transmittal.

Format


The form is provided in Microsoft Word in order to facilitate any changes to its structure or textual content. It is also fielded and tabbable for convenience. In making any non-fielded alterations, it will be necessary to select the "Unprotect" option from the "Tools" menu of your application. It can thereafter be protected again as a "Form" to regain the fielding.

Instructions


Reference - Include the official number if already assigned, the hull identification number, and vessel name. The name is not required but will help identify the vessel.

Contact Person - Include the name and mailing address of the sender. This does not have to match the owner's name or address if submitted by an authorized agent. Include the sender's email and phone number as applicable.

Items to Process - List all of the items being submitted, followed by the calculated fees if applicable. These can be determined from WikiLinks.

Items to Follow - List any related items that will be forwarded subsequent to the transmittal.

Please Provide - Request certified copies or other items.

Payment - Indicate the method of payment and include it in the transmittal.

Messages - Any special notes or instructions.

Date and Signature - If signing as a agent, be sure to indicate such capacity. Notarization is not required for the transmittal.

Conclusion
